To All Students Scheduled to Complete the Master’s Program

We will confirm the name notation for the degree certificate for students scheduled to complete the Master’s program in March 2026.

Please complete the following form by Monday, December 1, 5:00 PM (JST). This applies only to students scheduled to complete the Master’s program in March 2026:

If we do not receive your response by the deadline, the degree certificate will be prepared based on the information currently registered in the system.

Please note that requests for corrections after the deadline will not be accepted.
